2.  You need intermittent skilled care This includes skilled nursing, physical therapy, occupational therapy, or speech therapy. Care is part-time or intermittent, not 24/7 continuous care.

3.  You are under the care of a physician (or allowed provider) Your doctor establishes and periodically reviews the home health plan of care. A face-to-face encounter is typically required.
