What is CMS ACCESS model?

The Centers for Medicare & Medicaid Services (CMS) has launched a new resource to help healthcare professionals learn about the innovative ACCESS Model, a program designed to improve care for patients living with chronic conditions. This model supports individuals managing diabetes, high blood pressure, chronic musculoskeletal pain, and depression through technology-enabled care solutions.


What is CMS ACCESS model?- Beginning July 5, eligible patients can access a range of technology-supported disease management services at low or no additional cost. These services may include remote patient monitoring, wearable health devices, health coaching, lifestyle support programs, and medication management tools. The goal is to help patients better manage their health between appointments while improving outcomes and quality of life.


For primary care providers, community health centers, pharmacists, and other clinicians, the CMS ACCESS Model offers new opportunities to enhance patient engagement and strengthen care delivery. The model is designed to complement existing clinical services by promoting care coordination, improving communication between healthcare teams, and supporting patients with ongoing chronic disease management.
